Title: Taking redistribution principles into account in health economic evaluation: the Health Equivalent Income approach
Abstract: Various approaches are currently being explored at the international level to incorporate principles of redistributive justice into health economic evaluation (Cookson et al. 2020; Coast et al. 2008; Flynn et al. 2015). One such avenue involves evaluating interventions from the perspective of equalizing fundamental social outcomes (Fleurbaey, 1995) using an original well-being index, Health Equivalent Income (Fleurbaey, 2007). This approach is particularly relevant in situations where redistributive issues are important: for example, when treatment costs are high, when social determinants are associated with the onset and detection of disease, or when disease is likely to have an impact on the professional careers and therefore the incomes of the individuals concerned. These dimensions are only partially taken into account in standard cost/QALY approaches. Various research projects have been undertaken to demonstrate the feasibility of the approach and facilitate its adoption by evaluators. A first set of research projects aims to conduct surveys to estimate individual trade-offs in terms of health and income. A second set of research projects aims to develop modeling tools adapted to the theoretical prerequisites of the approach.
